KYC Mandate for Customers:
Effective 1st January 2023, submission of KYC documents is mandatory for all customers as per master guidelines on Anti Money Laundering/ Counter Financing of Terrorism (AML/CFT), 2022
Following documents are MANDATORY at the time of applying for a health insurance policy
- Identity proof
- Photograph
- Address Proof
PAN Number of the proposer need to be furnished in the application form
Failure to submit required KYC documents will lead to non-acceptance of proposal and non-issuance of policy
List of Mandatory Documents for KYC Process
As per new AML Regulation, submission of KYC documents is mandatory for all customers w.e.f 1st Jan’23
Mandatory Documents
- PAN/ Form 60
Valid ID proof (any one of the following)
- Aadhar Card
- PAN
- Passport
- Voter ID Card
- Driver’s License
- Job Card issued by NREGA signed by Govt. Officer
- Letter issued by National Population Register
- Letter from Gazetted Officer with duly attested photograph of customer
Valid Address Proof (any one of the following)
- Aadhar Card
- Passport
- Voter ID Card
- Driver’s License
- Recent Telephone Bill/ Post-paid mobile bill (not older than 2 months)
- Bank Passbook/Bank Statement
- Utility Bill (Electricity, Piped Gas, Water Bill – not older than 2 months)
- Accommodation allotment letter from the government
- Property or Municipal tax receipt
Photo Required
- Passport-size photograph
Note: All retail indemnity new business proposal buying Sum Insured >2.5Cr will be required to submit latest “Income tax return” proof along with KYC documents for enhanced due diligence purpose
