KYC Mandate for Customers:

Effective 1st January 2023, submission of KYC documents is mandatory for all customers as per master guidelines on Anti Money Laundering/ Counter Financing of Terrorism (AML/CFT), 2022

Following documents are MANDATORY at the time of applying for a health insurance policy

  1. Identity proof
  2. Photograph
  3. Address Proof

PAN Number of the proposer need to be furnished in the application form

Failure to submit required KYC documents will lead to non-acceptance of proposal and non-issuance of policy

List of Mandatory Documents for KYC Process

As per new AML Regulation, submission of KYC documents is mandatory for all customers w.e.f 1st Jan’23

Mandatory Documents

  1. PAN/ Form 60

Valid ID proof (any one of the following)

  1. Aadhar Card
  2. PAN
  3. Passport
  4. Voter ID Card
  5. Driver’s License
  6. Job Card issued by NREGA signed by Govt. Officer
  7. Letter issued by National Population Register
  8. Letter from Gazetted Officer with duly attested photograph of customer

Valid Address Proof (any one of the following)

  1. Aadhar Card
  2. Passport
  3. Voter ID Card
  4. Driver’s License
  5. Recent Telephone Bill/ Post-paid mobile bill (not older than 2 months)
  6. Bank Passbook/Bank Statement
  7. Utility Bill (Electricity, Piped Gas, Water Bill – not older than 2 months)
  8. Accommodation allotment letter from the government
  9. Property or Municipal tax receipt

Photo Required

  1. Passport-size photograph

Note: All retail indemnity new business proposal buying Sum Insured >2.5Cr will be required to submit latest “Income tax return” proof along with KYC documents for enhanced due diligence purpose
